Inclusion Health Services: Co-occurring Conditions Support Services SERVICE MODEL: Directly support or signpost people experiencing multiple disadvantage. Multiple Disadvantage is defined as adults experiencing a combination of - homelessness, substance misuse, mental ill health, domestic abuse and contact with the criminal justice system.
